One thing to know. These are general targets for healthy people. If you already have heart disease, diabetes, or several risk factors, your doctor will aim lower. Which LDL target applies to you personally?

This is where the general table above stops being useful. Singapore actually publishes the numbers, and they're worth knowing before your next appointment.

The Academy of Medicine Singapore Clinical Practice Guidelines on the Management of Lipids, published December 2023 by the Chapter of Cardiologists, sorts people into five risk groups and gives each one a different LDL target. It was written alongside the MOH Agency for Care Effectiveness lipid guidance, and the two were deliberately aligned. Here's how the targets fall:

  • Very high risk (established atherosclerotic cardiovascular disease). LDL below 1.8 mmol/L (70 mg/dL). If you've had an acute coronary syndrome, the guideline recommends going lower still, below 1.4 mmol/L (55 mg/dL).
  • High risk. LDL below 1.8 mmol/L (70 mg/dL). Long-standing diabetes sits here too. If you've had diabetes more than 10 years, or your glucose stays above target despite treatment, the same 1.8 applies.
  • Intermediate risk. LDL below 2.6 mmol/L (100 mg/dL).
  • Borderline risk. LDL below 3.4 mmol/L (130 mg/dL) can be considered.
  • Low risk. Lifestyle first. The guideline suggests a statin conversation only if LDL stays above 4.1 mmol/L (160 mg/dL), with 3.4 mmol/L as an ideal.

Look at the spread for a second. The very high risk target of 1.8 is well under the 2.6 that gets called desirable on a standard lab report. So a result printed as normal can be nowhere near normal for you. That's the single most useful thing to understand about cholesterol, and it's the reason two people can get identical numbers back and receive completely different advice.

Which group you land in isn't a guess. For people without existing heart disease it's calculated from the SG-FRS-2023 risk score, which weighs your age, sex, blood pressure, smoking status, diabetes and lipids together. Your doctor runs it. How do cholesterol targets differ across Asia?

More than you'd expect, and the gap matters if you've moved country or your doctor trained somewhere else. The same lab result can be called acceptable in one part of Asia and clearly too high in another.

Take the very high risk group, meaning people who already have atherosclerotic cardiovascular disease. Singapore's guideline puts the LDL goal below 1.8 mmol/L, which is 70 mg/dL. The Lipid Association of India's 2023 consensus statement puts the same group below 50 mg/dL. That's not a rounding difference. It's a substantially stricter bar for the same category of patient.

India goes further at the top end. Yashendra Sethi, Kunal Mahajan and Maciej Banach compared the major lipid guidelines side by side for a 2026 paper in Global Heart, the journal of the World Heart Federation. Their comparison shows the Lipid Association of India defining extreme risk categories with an optional goal of 30 mg/dL, and for a category C patient who keeps having events despite that, a target as low as 10 to 15 mg/dL. By contrast the same paper lists the European guideline's extreme risk goal at 40 mg/dL and notes the American guideline doesn't formally define an extreme risk tier at all.

So why would India set the strictest numbers in the world? Because the risk sitting behind a given LDL reading is different.   • Ask which guideline your doctor is working from. It's a normal question and it changes what your number means. In Singapore it's likely the Academy of Medicine guideline, in Malaysia the Malaysian dyslipidaemia guideline, in India often the Lipid Association of India.
  • Don't assume a target travels with you. If you were managed in one country and now see a doctor in another, bring your old results rather than your old target. The reading is the fact. The goal attached to it is a judgement call that varies.
  • Treat the stricter number as information, not an instruction. Seeing that India recommends 50 mg/dL doesn't mean you should be aiming there. It means the question of what's low enough genuinely isn't settled the same way everywhere, and it's worth discussing rather than assuming.

What do LDL, HDL, and triglycerides actually mean?

Cholesterol isn't one thing. It's a waxy substance your body needs, carried around your blood by different particles, and the type matters more than the total.

  • LDL, the bad one. Low-density lipoprotein drops cholesterol into your artery walls, where it builds up as plaque. Higher LDL means higher heart risk, which is why it's the main target for treatment.
  • HDL, the good one. High-density lipoprotein carries cholesterol back to the liver to be cleared. Higher HDL is generally protective, though very high levels don't add extra benefit.
  • Triglycerides. These are a fat in your blood tied to diet, weight, alcohol, and sugar. High triglycerides often travel with low HDL and belly fat, a pattern that's common across Asia.

You'll also see non-HDL cholesterol on some reports, which is your total minus HDL. It captures all the harmful particles in one figure, and many doctors now find it a better risk marker than LDL alone.

How common is high cholesterol in Asia?

More common every year, and that's the worrying part. For decades Asians had low cholesterol and low rates of heart disease. That's changing quickly. The NCD Risk Factor Collaboration, publishing in Nature in 2020, estimated that high non-HDL cholesterol was responsible for 3.9 million deaths worldwide in 2017, and that half of those happened in east, southeast, and south Asia. The geography flipped almost completely. In 1980 the countries with the highest levels were western European: Belgium, Finland, Iceland, Norway, Sweden, Switzerland. By 2018 the top of that list read Tokelau, Malaysia, the Philippines, and Thailand. High cholesterol stopped being a rich-Western problem and became an Asian one inside a single generation.

Country data tells the same story. Yang and colleagues surveyed 46,239 adults for the China National Diabetes and Metabolic Disorders Study and published the lipid results in Circulation in 2012. Age-standardised averages came out at 4.72 mmol/L total cholesterol, 2.68 mmol/L LDL, 1.30 mmol/L HDL, and 1.57 mmol/L triglycerides. Those are population averages, not patient averages, which is what makes them sobering.

South Korea shows how fast this moves. The Korean Society of Lipid and Atherosclerosis analysed KNHANES data from 2007 to 2020 for its 2022 Dyslipidemia Fact Sheet and found 40.2 percent of adults aged 20 and over had dyslipidemia. Hypercholesterolemia specifically hit 24.0 percent by 2020, up from 8.8 percent in 2007. That's close to a tripling in thirteen years, in one country, with no change in genetics. Rising incomes, more processed food, and less daily movement are doing this right across the region.

Singapore is a genuine exception, and it's worth saying so rather than pretending the whole region moves as one. The Ministry of Health's National Population Health Survey 2024, which interviewed residents aged 18 to 74 between July 2023 and June 2024, reports that about 1 in 3 Singapore residents still have hyperlipidaemia. The reported figure is 30.5 percent, down from 39.1 percent in the 2019 to 2020 survey. MOH is clear that prevalence has been falling over successive surveys. So the direction of travel here is the opposite of the regional trend. That's encouraging, and it's also a reminder that 1 in 3 is still a lot of people walking around with a condition that produces no symptoms whatsoever.

How many people don't know they have high cholesterol?

More than half of everyone who has it, at least in Malaysia, and that's the number that should make you book a test.

Malaysia runs a national health survey every four years, and the 2023 round is the best data anyone has on this. Mat Rifin, Mohd Yusoff, Abd Hamid and colleagues reported the results in Scientific Reports in July 2025, drawing on 13,616 people surveyed between July and September 2023 with an 83.2 percent response rate. Hypercholesterolaemia came in at 33.3 percent of Malaysian adults, or roughly one in three.

Now split that figure, because the split is the story. Only 15.2 percent were people who already knew they had it. The other 18.1 percent were found by the blood test on the day, in people with no idea their cholesterol was raised. So the undiagnosed group is bigger than the diagnosed group. For every Malaysian adult managing high cholesterol, there's slightly more than one walking around with it and no clue.

That happens because high cholesterol does nothing. No headache, no chest twinge, no fatigue, nothing you'd notice in the mirror. Blood pressure at least sometimes announces itself. Cholesterol never does. The damage accumulates quietly in your artery walls for years, and for a lot of people the first symptom is the heart attack.

Two useful things follow from this. First, feeling fine tells you nothing about your lipids, so don't use it as a reason to skip screening. Second, being slim tells you nothing either, which matters more here than in most parts of the world given how common normal-weight metabolic problems are across Asia. The only way to know is the blood test, and it's cheap, quick, and available at any polyclinic or GP.

How much does high cholesterol actually raise your risk?

Enough to take seriously, and the strongest Asian-specific evidence puts real numbers on it rather than leaving you to guess.

Nagasawa and colleagues pooled 10 Japanese cohort studies covering 65,594 people and published the analysis in the Journal of the American Heart Association in 2012. They broke the results down by sex and age, and that split is where it gets interesting.

For coronary heart disease, middle-aged men aged 40 to 69 in the highest total cholesterol band, 6.21 mmol/L and above, had 2.52 times the risk of men in the lowest band. Older men aged 70 to 89 came out at 2.77 times. Middle-aged women showed the strongest link of the four groups at 3.20 times. And older women showed essentially nothing, a hazard ratio of 1.02, which is just a technical way of saying no measurable difference at all.

Same number, different weight depending on who's carrying it. That theme keeps resurfacing in this data, and it's the main argument against treating any single threshold as universal.

Now the finding that surprises people, and the one to handle carefully. In that same pooled analysis, total cholesterol showed no association with cerebral infarction, the clot-type stroke. And it showed an inverse association with total stroke and with cerebral haemorrhage, the bleed type, meaning lower total cholesterol tracked with more haemorrhagic stroke rather than less. This pattern has turned up repeatedly in Japanese cohorts and it's a well-known wrinkle in the research.

But please don't read it as permission to relax about high cholesterol, and absolutely not as a reason to stop taking a statin. It isn't either of those things. What it genuinely shows is why one number can't carry a medical decision by itself. Your doctor weighs heart risk against stroke risk, alongside your blood pressure, diabetes status, smoking history, and family history, and reaches a judgement no lookup table can make for you. Why can your total cholesterol number mislead you?

Because total cholesterol adds together things that pull in opposite directions. It's HDL plus non-HDL in one figure, and HDL going up is generally good news while non-HDL going up is not. Two people can have the exact same total and be in completely different shape.

There's a large piece of evidence that makes this concrete, and it's specific to Asia. The NCD Risk Factor Collaboration pooled 458 population-based studies covering 82.1 million people across 23 countries and published the analysis in the International Journal of Epidemiology in February 2020. Total cholesterol rose in all four Asian countries they looked at. But the reason it rose was not the same everywhere.

In Japan and South Korea, the increase came mostly from HDL climbing, by 0.04 mmol/L per decade in South Korean men up to 0.17 mmol/L per decade in Japanese women. Their total-to-HDL ratio actually improved over the same period, even as the headline number went up. In China it was the opposite story: HDL barely moved in women, the rise came from non-HDL cholesterol, and the total-to-HDL ratio got worse. Same direction of travel on the number everyone quotes, two very different meanings underneath.

The researchers put it plainly, noting that the change in mean total-to-HDL ratio was only moderately correlated with the change in mean total cholesterol. So if your total comes back slightly raised, that alone doesn't tell you much. Ask your doctor for the breakdown, and pay particular attention to your non-HDL figure and your total-to-HDL ratio. Those two say far more about your heart than the headline number does.

Why does cholesterol carry different risks for Asians?

Two reasons stand out. First, body composition. Many Asians carry more visceral fat, the deep belly fat around your organs, at a lower body weight, and that pattern drives up triglycerides and drives down HDL. Second, and this is the part most guides flatten, "Asian" isn't one lipid profile. Frank and colleagues looked at 169,430 primary care patients aged 35 and over in northern California between 2008 and 2011, splitting the Asian American group into Asian Indian, Chinese, Filipino, Japanese, Korean, and Vietnamese instead of lumping them together. They published it in Circulation in February 2014. The subgroups came out looking genuinely different from each other. Asian Indians and Filipinos showed a higher prevalence of low HDL and high triglycerides. Chinese patients had lower LDL and triglycerides. Japanese patients had higher HDL than non-Hispanic whites. Asian Indian men, along with Asian Indian, Filipino, and Vietnamese women, stood out as the groups most likely to have combined dyslipidemia, meaning more than one lipid out of range at the same time.

So a low HDL reading carries different weight depending on where your family is from, and a normal-looking HDL can be falsely reassuring for some groups. Add in higher rates of diabetes at lower BMI, and the same cholesterol number can mean more heart risk for an Asian adult than for a Western one. Does LDL particle size matter more than the number?

Sometimes, yes. And this is the piece that explains why two people can walk out with the same LDL result and face genuinely different risk.

Your LDL reading measures how much cholesterol is being carried, not how many particles are carrying it or how big they are. Picture the same amount of cargo moved either by a few large trucks or by many small ones. The tonnage matches. The traffic doesn't. Small, dense LDL particles slip into the artery wall more easily, oxidise more readily, and stick around longer than large fluffy ones do.

That distinction matters a lot for South Asians. A 2025 review in Cardiovascular Diabetology, Endocrinology Reports set out the pattern plainly: South Asians carry smaller and denser LDL particles than white Europeans, those particles are more prone to aggregating in the artery wall, and LDL goes on to cause cardiovascular disease at lower serum levels than it does in white Europeans. The same review reported that hypertriglyceridaemia affects as much as 70 percent of the South Asian population, against roughly a third of white Europeans, and that HDL appears to work less protectively in South Asians even when the number looks fine.

So a South Asian adult can be told their LDL is acceptable and still be carrying meaningful risk. The number isn't wrong. It's just measuring the wrong thing for that person.

Mainland China shows a different version of the same lesson. The China Health and Nutrition Survey looked at 4,383 adults in 2015, with results published in Nutrients in 2019, and found 37.5 percent had dyslipidaemia. But look at how that broke down. Low HDL was the most common problem at 21.1 percent, followed by high triglycerides at 15.2 percent. High LDL came in at just 12.7 percent, and high total cholesterol at 9.6 percent.

Read that again, because it reframes the whole thing. Across much of Asia the dominant lipid problem isn't the one everybody talks about. It's low HDL and high triglycerides, the two numbers people tend to skim past on the way to checking their LDL.

What do you do with this? Don't try to eyeball particle size from a standard panel, because you can't. Does cholesterol change around menopause?

Yes, and the shift is sharper than most women are warned about. If your numbers jumped in your late forties or early fifties without any change to how you eat or move, this is very likely why.

The best evidence comes from SWAN, the Study of Women's Health Across the Nation, which enrolled 3,302 women aged 42 to 52 across seven US sites and has followed them ever since. It matters for this guide because the cohort isn't only White women. In the SWAN lipid substudy, 11.0 percent of participants were Chinese and 8.9 percent were Japanese, so the findings aren't being borrowed from a population that looks nothing like the readers of this page.

What SWAN found is that the change clusters tightly around one point in time. LDL cholesterol and apolipoprotein B both accelerate within about a year of the final menstrual period. Not a slow drift across the decade. A steep move in a narrow window, then a new higher baseline.

And there's a twist in the HDL numbers that's worth understanding, because it can make a report look reassuring when it isn't. Samar El Khoudary and colleagues published the SWAN-HDL analysis in Arteriosclerosis, Thrombosis, and Vascular Biology in 2020, following 471 women across up to 1,288 measurement points. HDL cholesterol went up through the transition. But the particles doing the work got worse at their job. Large HDL particles declined, small ones increased, and the amount of cholesterol each particle could actually shift out of the artery wall fell.

So a woman can come out of menopause with a higher HDL reading and less protection than she had before. That's the same lesson as the total cholesterol section above, just arriving from a different direction. The number went the "good" way and the biology went the other way.

Practically, this argues for getting a lipid panel done in your mid forties so you have a baseline from before the transition, and again after your periods stop. Comparing the two tells you far more than a single reading taken at either end. And if your numbers move, that's biology doing something predictable rather than evidence you've let yourself go.

What is Lp(a), and does a standard test miss it?

Yes, a standard lipid panel misses it. Lipoprotein(a), written Lp(a) and said out loud as "L P little a", is an LDL-like particle carrying an extra protein that makes it stickier and more likely to drive clotting. Your usual cholesterol test doesn't measure it. It has to be ordered separately, and most people never have it checked.

Here's why it belongs in a guide written for Asian adults. Your Lp(a) level is set almost entirely by the genes you inherited, so diet and exercise barely move it. And how common a high level is depends a lot on ancestry. A 2025 review in the Journal of the American Heart Association by Patel and colleagues reports that Lp(a) above 50 mg/dL shows up in roughly 25 percent of South Asian people, compared with about 20 percent of White and about 10 percent of East Asian people.

But the East Asian half of that sentence comes with a real catch, and it's the most useful thing on this page if your family is Chinese, Korean, or Japanese. Lower average levels don't mean the threshold is the same. Kim and Kim reviewed the evidence for the Journal of Lipid and Atherosclerosis in 2025 and found East Asians have the lowest median Lp(a) in the world, with UK Biobank putting Chinese participants at 16 nmol/L against 31 for South Asian and 19 for White participants. Yet a Korean cohort found cardiovascular risk climbing from about 26.3 mg/dL, well under the 50 mg/dL cutoff used in Western guidance. Their conclusion was that even modest elevations track with risk in Asian populations, and that one universal cutoff may not fit everyone.

The South Asian numbers are the starkest. That same review found people of South Asian background under 50 had the strongest link to early heart disease of any group examined, with an odds ratio of 3.71 and a confidence interval running from 2.31 to 5.96. Which helps explain something families notice long before a study does: heart attacks arriving a decade earlier than they seem to elsewhere.

What to do with this is refreshingly simple. Lp(a) is thought to be worth measuring roughly once in a lifetime, because it doesn't drift much. The Lipid Association of India recommends a single measurement in all South Asian adults to help work out risk, and the JAHA authors argue once-per-lifetime testing should sit inside routine lipid panels, particularly for people of South Asian ancestry.

A few honest caveats before you go asking. It isn't part of standard screening in most of Asia, you may have to pay for it, and there's currently no medicine approved specifically to lower Lp(a), though several are in trials. Knowing the number doesn't fix it. What it does is change how hard you and your doctor push on everything you can change, especially your LDL. So this is a question to raise at your next appointment, not a reason to worry tonight, and it matters most if heart disease has turned up early in your family.

Should you ask for an ApoB test?

Possibly, and it's the test most likely to change how your risk gets read. ApoB stands for apolipoprotein B, and it answers a different question from the one your LDL number answers.

Here's the distinction, because it's simpler than it sounds. LDL cholesterol tells you how much cholesterol is being carried. ApoB tells you how many particles are carrying it. Every particle that can lodge in an artery wall has exactly one ApoB molecule stuck to it, so counting ApoB counts the vehicles rather than the cargo. Two people can have identical LDL numbers while one is running many small particles and the other a few large ones. The one with more particles is generally at higher risk, and their standard panel won't say so.

This gap between the two markers is called discordance, and it's been measured in an Asian population. Jia and colleagues published a study in Frontiers in Cardiovascular Medicine in 2022 following 5,279 Chinese adults who were free of cardiovascular disease at the start. Among people whose LDL cholesterol sat below the median, in other words people whose results looked fine, 7.9 percent had discordantly high ApoB and 6.2 percent had discordantly high non-HDL cholesterol.

That mattered. Those with low LDL but high ApoB had an odds ratio of 1.40 for increased arterial stiffness, and the low LDL with high non-HDL group came in at 1.56. Roughly one in twelve people with a reassuring LDL result was carrying risk the panel didn't show.

But the same study is a good reminder not to treat any single marker as the winner. When the researchers looked at carotid artery wall thickness rather than stiffness, LDL cholesterol was the better predictor, with people who had high LDL but discordantly low ApoB showing an odds ratio of 1.74. Their conclusion was split: ApoB and non-HDL read arterial stiffness better, LDL read artery wall thickening better. So ApoB adds information. It doesn't replace anything.

Where it's most worth asking about:

  • Your triglycerides run high. This is when LDL and ApoB drift apart most, and high triglycerides are common across Asia.

  • You're already on treatment and want to know if it's enough. ApoB is useful for checking residual risk once LDL is at target.

It's a cheap, ordinary blood test and it can be added to a normal panel. Targets vary between guidelines and by your personal risk level, so ask your doctor what number applies to you rather than reading a threshold off the internet. How can you lower your cholesterol naturally?

Diet and lifestyle move cholesterol more than most people expect, especially LDL and triglycerides. These are the best-supported steps:

  • Swap the fats. Cut saturated fat from fatty meat, coconut milk, palm oil, and fried food, and avoid trans fats entirely. Replace them with unsaturated fats from nuts, seeds, and vegetable oils.
  • Eat more soluble fibre. Oats, beans, lentils, barley, and plenty of vegetables bind cholesterol in the gut. Just a few grams a day can lower LDL.
  • Add oily fish. Salmon, mackerel, and sardines bring omega-3s that help lower triglycerides. Aim for two servings a week.
  • Move most days. Around 150 minutes of moderate activity a week raises HDL and lowers triglycerides. See our exercise recommendations for Asian adults.
  • Lose excess weight and stop smoking. Even a modest drop in weight improves your whole lipid profile, and quitting smoking raises HDL within weeks.

These changes work best together, and they work alongside medication, not instead of it. If your doctor has prescribed a statin, keep taking it unless you're told otherwise. Lifestyle and medicine are partners here.

When should you get your cholesterol checked?

Because high cholesterol has zero symptoms, a blood test is the only way to know your numbers. Most healthy adults should get a lipid panel every few years from around age 40, and earlier if you have a family history of heart disease, diabetes, high blood pressure, or you're carrying extra weight around the middle. In Singapore, cholesterol screening is part of routine health checks from age 40, sometimes sooner.

And there's a newer Singapore programme worth knowing about if your reading comes back very high. On 30 June 2025 the Ministry of Health launched a national familial hypercholesterolaemia genetic testing programme. Familial hypercholesterolaemia, or FH, is an inherited condition that keeps LDL high from birth rather than from anything you ate, and MOH estimates roughly 20,000 people in Singapore have it. If your LDL-C comes back at 5.5 mmol/L (212 mg/dL) or higher, your doctor can refer you, and Singapore Citizens and Permanent Residents get means-tested subsidies of up to 70 percent alongside MediSave support. This one matters more than it sounds. FH barely budges with diet changes alone, and identifying it means your parents, siblings, and children can get checked as well.

If your numbers come back high, don't panic, but don't ignore them either. Book a proper follow-up. A doctor can look at your full risk picture, not just one number, and decide whether lifestyle changes alone are enough or whether medication makes sense. Treating cholesterol early is one of the most effective things you can do for your heart.

What happens if your cholesterol comes back high?

Your doctor works out your overall cardiovascular risk first, then sets a target from there. The number they aim for isn't the same for everyone, and that's the part most people get wrong when they compare results with a friend.

Malaysia spells this out clearly. The Clinical Practice Guidelines on Management of Dyslipidaemia, 6th edition, published in 2023 by the Ministry of Health Malaysia with the Academy of Medicine and the National Heart Association of Malaysia, ladders LDL targets by risk. No risk factors puts you below 4.2 mmol/L (160 mg/dL). One or more risk factors tightens it to below 3.4 mmol/L (130 mg/dL). Established cardiovascular disease, occlusive arterial disease, or diabetes brings it to below 2.6 mmol/L (100 mg/dL), and very high risk pushes toward below 1.8 mmol/L (70 mg/dL). Singapore's risk-based approach works the same way. So an LDL of 3.0 mmol/L might be fine for one person and a problem for another sitting in the same waiting room.

Here's the uncomfortable finding, though, and it's the reason this section exists. Knowing the targets isn't the bottleneck. Getting people onto treatment is.

Razman, Baharudin, Mohd Kasim and colleagues screened 5,279 people in the Malaysian community and published in Healthcare in 2022. Among participants at very high cardiovascular risk, only 15.9 percent had reached their LDL-C target, and 75.8 percent weren't on any lipid-lowering therapy at all. In the high-risk group it was 16.1 percent at target and 87.7 percent untreated. Read that again. Close to nine in ten high-risk people, the ones guidelines say should almost certainly be on medication, weren't taking anything.

It's a regional pattern, not a Malaysian one. Lee, Llanes, Sukmawan and colleagues reviewed lipid disorder data across the Asia-Pacific for Lipids in Health and Disease in 2021. In the CEPHEUS study, 49.1 percent of Asian patients on lipid-lowering treatment had reached goal, but the spread between countries was enormous, from 82.9 percent in Hong Kong down to 31.3 percent in Indonesia. Their review also shows how differently the problem presents around the region. Low HDL reached 71 percent in the Philippines and 67 to 72 percent in Vietnam, against 23.1 percent in Australia. High total cholesterol ran from 30.2 percent in Vietnam to 47.7 percent in Malaysia.

What you should take from all this is practical. A high reading is the start of a conversation, not the end of one. If your doctor recommends a statin, the evidence says the bigger risk for most people in this region is not taking it rather than taking it. And if you've been given a target, ask what yours is specifically and what your last number was, because "a bit high" isn't something you can act on. Side effects are worth raising too. They're usually manageable, and there are alternatives if one drug doesn't suit you, but stopping quietly on your own is the option that tends to go badly.

There's one version of a high reading that deserves its own paragraph, because the usual advice doesn't apply to it and most people have never heard of it.

If your LDL is very high, has been high since you were young, and hasn't shifted much no matter what you eat, that pattern can point to familial hypercholesterolaemia. FH is inherited, not earned. The liver's ability to clear LDL is impaired from birth, so the number stays high on a genuinely good diet, which is why people with it often spend years being told to try harder at something that was never going to work.

It isn't rare either. Singapore's Ministry of Health, launching a national genetic testing programme on 30 June 2025, put the figure at an estimated 20,000 people in Singapore, and said those affected face a risk of heart attack at a younger age that runs up to 20 times higher than the general population. Globally, the European Atherosclerosis Society has long described FH as underdiagnosed and undertreated, with well under 1 percent of cases identified in most countries.

The Singapore programme runs through Genomic Assessment Centres, starting at the National Heart Centre, and covers counselling before the test, the genetic test itself, and counselling afterwards. Eligible citizens and permanent residents get subsidies of up to 70 percent.

The part that makes this worth raising even if it turns out not to be you: FH travels through families. MOH notes that when someone tests positive, their parents, siblings and children are all at risk and are encouraged to get tested too. That's called cascade screening, and it means one diagnosis can end up protecting several people who had no idea.

So two things are worth mentioning to a doctor rather than trying to settle yourself. A persistently high LDL that diet and exercise haven't moved. And a family history of heart attacks or strokes before around 55 in men or 65 in women. Neither proves anything on its own. Both are fair reasons to ask the question.

What should happen to your cholesterol after a heart attack?

Everything speeds up. The targets get stricter, the clock starts running, and the old approach of trying one drug and reviewing in six months stops being good enough.

This is the newest guidance in this whole guide, and it's worth knowing wherever in Asia you are. Ru San Tan, Jonathan Yap, Colin Yeo, Kian Keong Poh and colleagues ran a modified Delphi study with a ten-member expert panel, published in the Annals of the Academy of Medicine, Singapore in June 2026. It asked what should actually happen after an acute coronary syndrome, which covers heart attacks and unstable angina.

Several points reached 100% agreement across the panel, which is unusual and tells you these aren't close calls:

  • Two targets, not one. Cut LDL by at least 50% from your starting level and get below 1.4 mmol/L (55 mg/dL). Hitting one without the other isn't the goal.
  • Test within 24 hours of admission. That baseline is what the 50% reduction gets measured against, so missing it makes the target harder to judge later.
  • Recheck at 4 to 6 weeks. Not at the next annual review.
  • Speed appears to matter. The panel agreed unanimously that reaching target within 4 to 12 weeks is associated with lower risk of another major cardiac event.
  • Start with two drugs when one won't be enough. Rather than trying a statin alone and escalating months later, the panel backed beginning with a statin plus ezetimibe when statin monotherapy is unlikely to reach the target.

Two further points got 78% agreement rather than unanimity, so hold them a bit more loosely. If you've had a second event within two years, the panel would consider pushing below 1.0 mmol/L (40 mg/dL). And they'd consider a PCSK9 inhibitor at the point of discharge if your LDL is still above 1.4 mmol/L, rather than waiting to see how the statin does first.

The reason any of this exists is the gap described earlier in this guide. The panel noted that suboptimal lipid management after ACS remains common, which lines up with the regional screening data showing how few high-risk people are at target. Guidelines have said 1.4 mmol/L for years. What's changed is the recognition that how fast you get there is part of the treatment, not an administrative detail.

What to do with this if it applies to you or someone in your family. Ask what your admission LDL was, ask what the target is in actual numbers, and ask when the recheck is booked. Those three questions cover most of what the consensus is trying to fix. And if you're offered two medications at once rather than one, that's not your doctor being aggressive. It's current thinking. Do Asian adults need different statin doses?

Often yes, at least when starting out. And this isn't a fringe theory. It's printed on the drug label.

Rosuvastatin is one of the most widely prescribed statins in the region. Its US prescribing information, published on the National Library of Medicine's DailyMed database, tells prescribers this directly: "In Asian patients, consider initiation of rosuvastatin tablets therapy with 5 mg once daily." The reason given is that "pharmacokinetic studies have demonstrated an approximate 2-fold increase in median exposure to rosuvastatin in Asian subjects when compared with Caucasian controls." The label adds that this raised exposure should be taken into account for anyone not adequately controlled at doses up to 20 mg a day.

Read that plainly. The same tablet delivers roughly twice the drug exposure. Not twice the benefit necessarily, but twice the amount circulating.

Why the same dose behaves differently. Mostly genetics affecting how the liver takes the drug up and clears it. Chan, Coriolan and Plakogiannis reviewed this for the National Lipid Association's LipidSpin in 2021. The ABCG2 c.421C>A variant, which slows statin clearance, appears in about 35 percent of East Asians compared with 14 percent of Caucasians. Variants in SLCO1B1, the gene behind the liver's main statin transporter, push in the same direction.

You can see it in the trial results too. In the Japanese Lipid Intervention Trial, simvastatin at just 5 mg daily lowered LDL by 26.8 percent, roughly what 20 mg achieves in Western populations. In the DISCOVERY-Hong Kong trial, Chinese patients on rosuvastatin 10 mg saw LDL fall 52.8 percent, against a range of about 40.9 to 49.7 percent reported at the same dose in Western groups. Lower dose, similar or better result.

What to actually do with this. Here's where care matters, because this finding is easy to misread in a way that could hurt you.

  • Do not reduce or stop a statin on your own. Nothing above is a reason to change what you're taking. The dominant problem across this region is people not being treated at all, not people being overtreated. Remember the Malaysian community screening earlier on this page, where 87.7 percent of high-risk people were on nothing.
  • If you're starting treatment, ask what dose and why. "Is this a suitable starting dose for me?" is a completely reasonable question, and your doctor has probably already factored it in.
  • If you had side effects and gave up, go back. This is the group this research helps most. Muscle aches on a standard dose don't mean statins are impossible for you. A lower dose or a different statin often works, and quietly stopping is the outcome with the real risk attached.
  • Mention your other medications. Some drugs raise statin levels further, which matters more when your baseline exposure is already higher.

The honest summary is that dosing is your doctor's call, made with your full risk picture in front of them. What's useful for you to know is that if a standard dose felt rough, there's a documented reason that might explain it, and a conversation worth having instead of abandoning treatment altogether.

Are muscle aches on a statin actually from the statin?

Often not, and the evidence for that is stranger than you'd expect. But this needs saying carefully, because the finding is easy to hear as "your pain is imaginary," and that is not what the research says.

It matters because muscle aches are the single most common reason people quit a cholesterol drug. And quitting quietly, without telling anyone, is the worst available outcome.

What happens when you test it properly

Emily Herrett and colleagues ran the cleanest version of this experiment, published in the BMJ in 2021 as StatinWISE. They recruited 200 people across 50 UK primary care sites, all of whom had recently stopped statins or were about to, because of muscle symptoms. Not a general population. People who already knew statins hurt them.

Each person then became their own trial. Six two-month periods, atorvastatin 20 mg or a matching placebo, double-blinded, in random order. Nobody knew which month was which.

The result: "Overall, no difference in muscle symptom scores was found between the statin and placebo periods (mean difference statin minus placebo -0.11, 95% CI -0.36 to 0.14; P=0.40)."

And here's the part that says most about what people did with that information. "Two thirds of those completing the trial reported restarting long term treatment with statins."

A second trial, SAMSON, added a third condition that makes the mechanism visible. Participants cycled through months of statin, months of placebo, and months of an empty bottle. Symptom scores on the statin and on the placebo were similar. On nothing at all, they were substantially lower.

Real symptoms, wrong culprit

The International Lipid Expert Panel put the interpretation precisely in a 2022 position paper in the Journal of Cachexia, Sarcopenia and Muscle: the SAMSON pattern shows that "real symptoms result from the action of taking tablets but that the symptoms are not caused by the pharmacological agent."

Read that twice. The aches are real. They're produced by the act of taking a daily tablet you're worried about, not by the chemistry inside it. That's a completely different claim from saying you made it up.

The same panel puts a range on it. Reviewing trials that compared open-label with blinded statin use, they report that "between 38% and 78% of SAMS-related statin intolerance could be attributed to expectation alone." A wide range, and they're upfront that it's wide. They also separate the nocebo effect, meaning harm from something inert, from what they call the drucebo effect, the gap between taking a drug knowingly and taking it blind.

Why this lands differently in Asia

Both trials used atorvastatin 20 mg, in UK primary care. Neither one tells you what happens at a dose that is effectively higher for you than for the people studied.

The section above already established why that matters here. Rosuvastatin exposure runs roughly twice as high in Asian patients as in Caucasian ones, which is why the label suggests starting at 5 mg. The ABCG2 variant that slows statin clearance appears in around 35 percent of East Asians against 14 percent of Caucasians.

So the honest reading for an Asian adult isn't "it's all nocebo, keep going regardless." It's that dose and drug choice are genuinely adjustable variables, more so than the UK trials alone would suggest, and that adjusting them is a conversation with a doctor rather than a reason to stop.

What specialists suggest trying instead

The ILEP panel sets out an approach they abbreviate as SLAP for people who can't tolerate their current regimen:

  • Switch statins. They aren't interchangeable, and people who react to one often do fine on another.
  • Lower the dose. A reduced dose that you actually take beats a target dose you abandon.
  • Alternate-day dosing. Less frequent, still meaningful.
  • Polypharmacy. Adding a non-statin agent so a smaller statin dose still gets you to target.

Their wider framework, MEDS, starts with minimising disruption to lipid-lowering treatment, then educating patients about the proven benefits, using diet and nutraceuticals alongside rather than instead, and monitoring both symptoms and biomarkers.

Notice what appears on neither list. Stopping and saying nothing.

One important limit. These trials studied ordinary muscle aches, and both excluded people with severe reactions. Severe unexplained muscle pain, marked weakness, or dark urine are a different situation entirely and need prompt medical attention rather than a wait-and-see approach. For most adults, a desirable total cholesterol is below 5.2 mmol/L (200 mg/dL), LDL below 2.6 mmol/L (100 mg/dL), HDL above 1.0 mmol/L (40 mg/dL) for men or 1.3 mmol/L (50 mg/dL) for women, and triglycerides below 1.7 mmol/L (150 mg/dL). The protocol also advises that if you are already on a statin with an LDL under 2.1 mmol/L (80 mg/dL) when you turn 75, there is no need to reduce treatment that you are tolerating well.

How common is high cholesterol in Asia?

It is rising fast. The NCD Risk Factor Collaboration reported in Nature in 2020 that high non-HDL cholesterol was responsible for 3.9 million deaths worldwide in 2017, half of them in east, southeast, and south Asia. In South Korea, the Korean Society of Lipid and Atherosclerosis found 40.2 percent of adults aged 20 and over had dyslipidemia, with hypercholesterolemia reaching 24.0 percent by 2020, up from 8.8 percent in 2007. Cholesterol is climbing across the region even as it falls in the West.

How can you lower cholesterol without medication?

Cut saturated and trans fats, eat more soluble fibre from oats, beans, and vegetables, add oily fish, move around 150 minutes a week, lose excess weight, and stop smoking. These steps can meaningfully lower LDL. Lifestyle changes work alongside any medication your doctor prescribes, not instead of it, so keep taking prescribed statins unless told otherwise.
